Mountain Valley Home for Health, a service of Mountain Valley Regional Rehabilitation Hospital, has won the 2018 Press Ganey Guardian of Excellence Award. Press Ganey is an industry leader in measuring, understanding and improving the patient experience.
Mountain Valley Home for Health earned the distinction by achieving the 95th percentile in performance for Patient Experience during each reporting period in 2017. The award is presented annually.
“The Press Ganey Guardian of Excellence Award is a nationally-recognized symbol of achievement in health care,” said Mountain Valley Regional Rehabilitation Hospital Chief Executive Officer Judy Baum. “We’re proud to have been honored for providing safe, high-quality, patient-centered care. We’re passionate about providing this level of care to the communities we serve.”
Members of the Mountain Valley Home for Health team provide continued clinical care and specialized therapies to patients as they progress from inpatient hospital stays to their homes. Specialized services include:
- Stroke and brain injury rehabilitation
- Neurological rehabilitation, including Parkinson’s disease and multiple sclerosis
- Orthopedic and cardiac surgery aftercare
- Disease management, including diabetes, congestive heart failure, COPD
- Fall prevention and home safety
- Medication assessment, instruction and management
- Wound care
- Intravenous therapy
The team includes registered nurses, physical therapists, occupational therapists, speech-language pathologists, medical social workers and home health aides. QCBN
