Integrated care clinic services include primary care, adult outpatient services, peer support and counseling.
Polara Health has opened its new Integrated Care Clinic on Gail Gardener Way in Prescott, designed to provide comprehensive services that address medical, mental and social needs.
We provide life-changing power with the services that we provide,” said Marketing Director Deanna Eder. “Our commitment to the whole person means that we focus on restoring health, promoting resilience and preventing disease by addressing physical, behavioral, emotional and social aspects of well-being.
Polara Health has served those with mental health challenges for years but has recently began providing primary care. Integrated care clinic services include primary care, adult outpatient services, peer support and counseling.
The primary care doctors, Dr. Shurine Pate and Dr. Lisa Emmons, serve patients ages 18 and above and are taking new patients.
Eder notes that the office is not an urgent care center. However, she said Polara Health’s Prescott Valley facility offers crisis services around the clock to treat patients suffering from drug, alcohol or domestic abuse.
Polara Health, a nonprofit organization, believes it is critically important for everyone to be able to access whole-person care that improves well-being and quality of life.
“Everyone should have access to health care. We want people to feel cared for and we want them to feel comfortable,” said Eder. “We don’t want to turn anyone away. We want to meet their mental and physical needs and we are happy to provide medical services to the community.” QCBN
